For residential address, attach a copy of Identity Card/Aadhar Card/Driving License/Passport which mentions your correct residential address. For police verification, attach a valid online police verification certificate issued by the police department.
For affidavit – Attach the affidavit in the prescribed form – 49 on non-judicial stamp paper attested by a notary, along with your application form in an envelope with current address and pin code.
